Introduction
Welcome to Access Home Care Solutions (“Access,” “we,” “our,” or “us”). We provide home-based intrathecal pump refill services in partnership with licensed physicians and clinics. Our goal is to deliver safe, professional, and compassionate care in the comfort of your home.
These Terms and Conditions (“Terms”) outline the rules and responsibilities that apply when you receive services from Access. By scheduling or receiving our services, you agree to these Terms.
Scope of Services
Access Home Care Solutions provides the following services:

Intrathecal pump refills performed by qualified, licensed clinicians;

Monitoring, assessment, and documentation related to your pump refill procedure;

Coordination with your prescribing physician or clinic for ongoing treatment and medical oversight.
Access does not provide emergency medical services, primary medical care, or diagnostic services. All treatment decisions remain under the direction of your prescribing physician.
Eligibility for Services
To receive services from Access, you must:

Be under the active care of a physician who manages your intrathecal pump therapy;

Have a valid prescription or order for pump refills;

Provide accurate, up-to-date medical and contact information;

Ensure a safe and clean environment for the home visit.
Access reserves the right to decline or discontinue services if safety, regulatory, or ethical concerns arise.
Patient Responsibilities
You agree to:

Provide honest and complete health information;

Follow all care instructions provided by your Access clinician and physician;

Keep your scheduled appointment times or notify us promptly of cancellations;

Inform us immediately if you experience pain, discomfort, or unusual symptoms during or after the procedure;

Maintain a safe environment during the visit (including pets secured and adequate lighting).
Failure to follow instructions or provide accurate information may affect your treatment outcome and eligibility for continued services.
Coordination with Physicians and Clinics
Access works in collaboration with your physician or clinic to ensure continuity of care. Your physician retains full responsibility for your medical plan, prescriptions, and treatment decisions. Access clinicians perform pump refills as directed by your physician’s orders and document all procedures in compliance with state and federal regulations.
Consent to Treatment
By receiving services, you consent to:

The performance of intrathecal pump refill procedures in your home by qualified healthcare professionals;

The sharing of your health information, as permitted by law, with your prescribing physician, care team, or authorized insurance provider;

The use of electronic systems for scheduling, documentation, and secure communication.
You may withdraw consent at any time by notifying Access in writing. However, this may affect our ability to provide ongoing services.
Privacy and Confidentiality
Access respects your privacy and complies with all applicable HIPAA (Health Insurance Portability and Accountability Act) regulations.
Your personal and medical information will only be shared with authorized individuals involved in your care or as required by law.

Emergencies
Access is not an emergency medical service provider.
If you experience a medical emergency at any time, call 911 or go to the nearest emergency department.
If an urgent issue arises after your refill, please contact your physician or the on-call provider as instructed.
Compliance and Professional Standards
All Access clinicians are licensed and trained according to state and federal healthcare regulations.
Our services comply with the standards set by applicable state Boards of Nursing and the U.S. Department of Health and Human Services.
